Replacement Plate

Plates are replaced on vehicles when one or both plates have been lost or plates are defective.

Vehicles issued two plates

Complete a vehicle application form or provide written request for a replacement which includes:

  • plate number of vehicle (as displayed on the vehicle registration document) signature of registered owner
  • If both plates are missing, you must complete lost plate affidavit available at any Motor Registration office in your area.
  • If one plate is missing applicant must return remaining plate with the application to any Motor Registration office in your area;
  • If plates are defective applicant must return both plates to a Motor Registration Office. Defective plates are replaced at no charge, with supervisor authorization.

Vehicle issued one plate

Complete vehicle application form or provide written request which includes:
plate number of vehicle;
signature of registered owner; and

  • You must complete lost plate affidavit available at any Motor Registration office in your area.
  • If plate is defective applicant must return plate to a Motor Registration Office.
  • Defective plate is replaced at no charge, with supervisor authorization.

Replacement Sticker

  • You must complete a Lost Sticker Affidavit if sticker is lost or destroyed available at any Motor Registration office in your area.
  • Affidavit must be completed by registered owner.
  • If sticker is defective, applicant must return sticker and it will be replaced at no charge with supervisor authorization.
