  • The AFM Application for Student Access is submitted annually to AFM-NL.
  • The form for AFM Resources Required by Student is for school use and is not submitted to AFM-NL.
  • The Order Form to Request New Items from AFM-NL – The AFM advisor or Contact teacher submits this form to AFM-NL when a resource(s) is required that is not currently in the AFM-NL catalogue.

Application for AFM Advisor Access to the Alternate Format Materials Library NL(NLESD):

  • The Application for AFM Advisor Access is submitted once to AFM-NL.
Alternate Transportation Guidelines and Application
  • Student requires alternate transportation as determined by the IEP/ISSP team with medical support as per alternate Transportation Guidelines outlined on the application form
  • To be completed by the school as part of service delivery team planning
Assistive Technology Application
  • Student has an exceptionality which requires the use of assistive technology, either low tech or high tech, and the program planning team is seeking to obtain such technology from the Department of Education and Early Childhood Development
  • Application must follow the guidelines and process outlined including a detailed account of a thirty day educational trial of the technology requested.
  • The application is to be completed by members of the student’s program planning team.
Application for Home Teaching Services
  • Student is away from school for short term medical reasons and meets the criteria outlined on the application form
  • To be completed by the school
Home Teaching Services – Application for Fee
  • Used to request payment for the tutor assigned to a student through Home Tutoring Services
  • To be completed by the school administrator
HUSHUPS Application
  • To be completed by Teacher for Deaf or Hard of Hearing (DHH)
  • An application for HUSHUPS must be accompanied by a Listening Environment Profile
High School Bursary
  • Student is eligible to receive a bursary to attend another school outside of their community in order to receive sufficient credits for high school graduation
    • High School Bursary Guidelines (186 KB) (in their entirety, including the forms).
    • Student Bursary Application: Form 1 an application to be completed by student and signed by parent/guardian
    • Student Bursary Application: Form 2 an application to be completed by the Director of Education of the school district responsible for the home community school.
    • Bursary Monthly Report: Form 3 a report to be completed monthly by the school administrator.
