Service Delivery Model Forms

Educators implementing the Responsive Teaching and Learning (RTL) Policy do not use the Service Delivery Model (SDM) Guidelines or Forms

These forms guide the school team when developing the student’s program. The development of this program is the responsibility of the school. Parents/guardians are invited to provide input into the decisions regarding accommodations, modifications, or alternate programming. In the case of a student requiring support from other agencies, an Individual Support Services Plan (ISSP) may be required.
