Registration Now Available for Two New Collaborative Team Clinics

Residents in the Eastern Health region who have identified as being without a primary care provider (Family Doctor or Nurse Practitioner), can now register through Patient Connect NL to be placed on a list for the two new Collaborative Team Clinics opening in the St. John’s region in the new year.

Individuals may register online or by telephone at 1-833-913-4679. Information provided will assist in connecting residents with the right provider at the right time and the right place, based on their health and social needs. Once connected to a clinic, care would then be provided by a team of health care providers such as physicians, nurse practitioners, social workers, pharmacists and dieticians, depending on a patient’s needs. The Collaborative Team Clinics will access the listing from Patient Connect NL to connect residents with a clinic on a first come, first serve basis. For individuals who may wish to register multiple family members, the reference code provided upon the initial registration can be entered on additional registration forms, and every effort will be made to place all family members with the same clinic.

In October of this year, the Provincial Government announced a number of new initiatives to help strengthen the healthcare system, including interim steps to improve access to primary health care delivery while government awaits the recommendations from Health Accord NL expected early in the new year. These included the creation of two new clinics within the St. John’s region to complement the former clinic at Mundy Pond which has now relocated to Pippy Place. While registration is currently open to residents of the Eastern Health region only, it will be available in other regions as more Collaborative Team Clinics open throughout the province.
