It is now be easier for parents in Newfoundland and Labrador to apply for federal and provincial government benefits for their newborns. The new, integrated birth registration service bundles the Social Insurance Number (SIN), Canada child benefits, and Medical Care Plan (MCP).

  1. Registering the Birth of Your Child (MANDATORY)

    • The birth of every child in the province of Newfoundland and Labrador must be registered within 30 days of the date of birth through the completion of the enclosed Birth Registration Form.
    • The Birth Registration Form includes instructions for its completion on the reverse side.
    • The birth mother’s signature is required on the Birth Registration Form.
    • The other parent’s name may only be included on the birth registration if he / she also signs the form.
    • There is no charge for this registration.
    • A self-addressed stamped envelope is included for your use in returning this completed form to the Vital Statistics Division.
  2. Applying For a Birth Certificate (OPTIONAL)

    • A birth certificate, either in short or long form, can be obtained online or by completing the enclosed Application for Birth Certificate Form or apply online at www.gov.nl.ca/dgsnl
    • Guidelines for completion of the form are included on the back of the form.
    • The cost of a birth certificate is $35.00, $30.00 online.
    • Applicants are required to provide acceptable identification when applying for a birth certificate. If applying for your child’s certificate by mail, please include a copy of one piece of photo ID or two pieces of other ID (e.g. MCP; credit card), at least one of which contains your signature or address.
    • You may include this application in the same envelope as the birth registration form.
  3. Applying for Medical Care Plan (MCP) Beneficiary Card for your Child (OPTIONAL)

    • Application for a Medical Care Plan (MCP) Beneficiary Card for your child can be made by signing the Release of Information (section 6) of the Birth Registration Form.
    • MCP is administered by the Department of Health and Community Services and there is no charge for MCP coverage.
    • There is no fee for this application.
  4. Applying For a Social Insurance Number for Your Child (OPTIONAL)

    • Application for a Social Insurance Number (SIN) for your child can be made by signing the Release of Information (section 6) of the Birth Registration Form.
    • This optional service simplifies the process of obtaining a SIN for the purpose of opening a Registered Education Savings Plan (RESP) – and allows parents to apply for Government of Canada education savings initiatives including the Canada Learning Bond and the Canada Education Savings Grant.
    • The information will be sent to Service Canada after the birth is registered. Service Canada will then mail your child’s SIN to you.
    • Further information on the benefits of these programs is contained in the enclosed documents.
    • There is no fee for this application.
  5. Applying for Canada Child Benefits

    • Application for Canada child benefits (includes: Canada Child Benefit; the provincial Newfoundland and Labrador Child Benefit and Mother Baby Nutrition Supplement) can be made by signing the Release of Information (section 6) of the Birth Registration Form.

      • There is no fee for this application. This service is optional.
