If you received one or more doses of a World Health Organization approved COVID-19 vaccine in another province or country, you can upload your vaccination records to be reviewed and added to your Electronic Health Record.

In order to update your COVID-19 Vaccination Record, you will need:

• First and last name
• Date of birth
• An image of your official vaccination record confirming the details of your previous dose(s).
  • Your proof of vaccination must be in English. If your proof of vaccination is not in English, you will need a translation in English. Please submit both the original proof of vaccination and the translated version. If you require assistance finding a translator, please contact covid19info@gov.nl.ca.
  • If you were vaccinated in Alberta, your vaccination record must include a QR code. If you do not have a QR code, please contact the Government of Alberta.
• A valid Newfoundland and Labrador mailing address
• MCP number
