Description and Primary Use
Measurement of plasma ADH is not usually helpful for investigation of disorders of serum sodium concentration. Interpretation of plasma ADH levels requires knowledge of plasma osmolality and clinical context. Plasma ADH levels may have a role in the investigation of diabetes insipidus as part of a controlled water deprivation test. ADH promotes the reabsorption of water by the kidneys and is used to assist in diagnosing ADH deficiency (e.g. diabetes insipidus) or excess (e.g. syndrome of inappropriate ADH, (SIADH)).

Special Patient Preparation

To obtain basal levels, patient should fast for 12 hours and abstain from drinking fluids, including water, for 6 hours prior to testing (preferred). Testing should not be completed on patients who have recently received radioisotopes because of potential assay interference.

Special Collection / Transport / Processing Requirements

Specimen for this test must be collected, processed, aliquoted and frozen within 30 minutes of collection. Collect specimen in a pre-chilled EDTA tube and place immediately on ice. Pre-chill specimen for 10 minutes. Centrifuge. Aliquot plasma to a transport tube and freeze.

Criteria for Rejection

Grossly hemolyzed; not maintained frozen.
