Patient should bring sample to lab ASAP after collection. Testing can only be order by NLFS physicians after consultation with patient.

- Consultation with NLSF physician is required as well as signing a cryopreservation consent.

- Special Patient Preparation
At least 2 days of sexual abstience is reccomended, but no more then 7 days. Patient requires recent serology testing before banking.
- Special Collection / Transport / Processing Requirements
Patient is reccomended to collect on site but may collect at home. If collecting at home patient should drop off sample within 30 mins of collection. If patient in inpatient arrangements will be made ahead of time with HSC sorting and Fertility labs transportable incubator.
- Criteria for Rejection
Banking procedure may not proceed if patient has not collected serology testing. Banking will not proceed is no motile sperm are observed.
