Approval requires:
- Test ordered STAT from OR or by surgeon (during or post -parathyroid & thyroid surgery).
- Confirmed hypercalcemia or hypocalcemia.
- PTH monitoring in CKD patient.
- A completed LTSA form indicating:
- Monitoring Parathyroid disease (e.g. monitoring disease progression, bisphosphonate therapy, or Parathyroid cancer).
- Test directed by an endocrinologist (Identify the name of the specialist).
- High risk for parathyroid disease (e.g. High risk for secondary hyperparathyroidism.)
