Ordering Requirements (Restrictions / Conditions)

Approval requires:

  1. Test ordered STAT from OR or by surgeon (during or post -parathyroid & thyroid surgery).
  2. Confirmed hypercalcemia or hypocalcemia.
  3. PTH monitoring in CKD patient.
  4. A completed LTSA form indicating:
    1. Monitoring Parathyroid disease (e.g. monitoring disease progression, bisphosphonate therapy, or Parathyroid cancer).
    2. Test directed by an endocrinologist (Identify the name of the specialist).
    3. High risk for parathyroid disease (e.g. High risk for secondary hyperparathyroidism.)
