Restricted to investigation of patients:
- Receiving daily vitamin D supplementation of 2000 IU for 6 months;
- At high risk or being treated for Osteomalacia/Rickets;
- At high risk or being treated for Osteoporosis/Osteopenia;
- Unexplained hyper/hypocalcemia; or
- Seen by an endocrinologist (Must be ordered by endocrinologist)
A LTSA form must be completed with the test name clearly identified and one of the above clearly indicated as reason for the test request.

Collect in red top tube without separation gel. Centrifuge and transfer serum in a new tube and freeze at -20oC.
