Replacement Licence plate(s)
You can make an appointment to have a lost, stolen or destroyed licence plate replaced.
The registered owner of the vehicle must bring:
- A completed affidavit (where applicable)
- This form is available at any Motor Registration office
- The plate (if possible) and
- The vehicle registration
Staff will confirm the registered owner’s identity, update the vehicle records, and the plate(s) will be exchanged.
