body weight equations. Ann Pharmacother. 2000 Sep;34(9):1066-9. |

Ideal Body Weight (IBW), Devine Formula is: |
|
Ideal Body Weight (male) = 50.0 kg + 2.3 kg
(each inch > 5 feet)
Ideal Body Weight (female) = 45.5 kg + 2.3 kg (each inch >
5 feet)
|
[b] |
Dosing Weight is an adjusted body
weight (of obese or overweight patients) and should only be used to
calculate the dose of drugs for which there are recommendations
specifying that the actual body weight
should be adjusted for use in the dose calculations.
- Dosing Weight = IBW + [0.4 x (Actual
- IBW)]
-
exception: if Actual < IBW, then Dosing
Weight = Actual

Disclaimer:
The NL Provincial Blood Coordinating Program (PBCP) gratefully acknowledges the Ontario Regional Blood Coordinating Network (ORBCoN) for use of their template for the Ideal Body Weight Calculator with IVIG Dosing.
The NL PBCP has made the following modifications:
- There is no upper limit of actual weight for the dose calculations
- Dosage increments of 0.1 extend up to 1.0 gm/kg.
The Dose Calculator is intended to be used when determining the dose of IVIG for clinically obese patients. For other patients, the dosing information recommended in the Newfoundland and Labrador Utilization of IVIG policy and standard operating procedure for review and approval of requests for IVIG in adults should be followed.
Neither the guidelines nor the dose calculator is intended to replace sound clinical judgment concerning a patient's unique situation. Monitoring of IVIG use is ongoing and use of the dose calculator to limit overuse of the product is strongly encouraged.
Furthermore, although the advice and information contained in the guidelines and dose calculator is believed to be true and accurate at the time of going to press, neither the authors nor the publishers shall be liable for damages of any kind related to their use or misuse or errors or omissions that may have been made.
